Job Description:

Join a dedicated multidisciplinary care management team as a nursing case manager in Oklahoma City. In this role, you'll take on the vital task of coordinating comprehensive care for a carefully selected patient population. Your day will involve assessing patient goals across biophysical, psychosocial, environmental, discharge planning, and financial aspects. You'll keep a close eye on patient progress, ensuring any medical changes are promptly addressed to facilitate smooth transitions. Acting as an advocate, you'll navigate scarce resources and collaborate with third-party payers to resolve issues, all while coordinating discharge planning with families, healthcare teams, and community resources. Your expertise will help determine appropriate levels of care and ensure seamless patient transitions, making a measurable difference in their recovery journey.
