Job Description:

Join a dedicated team as a Case Manager RN in an inpatient acute care setting that cares for both adult and pediatric patients. This role involves managing a caseload of approximately 25 patients, coordinating care and facilitating patient transitions across a vibrant hospital environment. You'll work on a consistent Monday through Friday schedule, with 8-hour daytime shifts, providing stability and routine in your workday. This assignment offers a combination of clinical oversight and patient management, making it an engaging experience for those looking to impact patient paths without direct bedside nursing. You'll also have opportunities to collaborate across departments, ensuring effective discharge planning and resource utilization in a level I trauma, teaching hospital setting.
