Join a reputable healthcare facility in Boston as a case management professional, where you'll coordinate patient discharge planning for a busy acute care hospital. This role involves managing a caseload in a large, possibly teaching hospital setting, with shifts typically consisting of 10-hour days, Monday through Friday.
