Flu Season Information

  • To ensure your documents are received with enough time for review, please be sure to upload to your My Atlas Access. Do NOT email as we will need them uploaded directly to your portal.
  • Upload no later than 9/27 or sooner based on current assignment requirements.

 

Flu Vaccination Documentation – REQUIRED

  • You must have the pharmacy print the Vaccine Administration Form or transcribe the Atlas Flu Consent Form. Atlas MedStaff requires the following:
    • Download your consent form here
    • Your flu shot documentation must include the following:
      • Your name
      • Date given (must be current flu season)
      • Signature/name of person that gave vaccination
      • Lot # and expiration date
      • Site given (right/left arm)
      • Clinic name and address
  • Atlas MedStaff will not accept Rx receipts, stickers/cards, or screenshots as acceptable documentation

 

Flu Vaccination Form – REQUIRED

  • You will be assigned a General Flu Attestation (aka Flu Decline) form via your My Atlas Access.  These forms are REQUIRED to be completed on an annual basis
    • During Flu Season this form can be utilized in lieu of the vaccine being on file for Atlas purposes.
    • The Flu Declination form must be on file at all times
      • If we do not have a flu vaccine record on file, we will utilize the flu form to expire seasonally (10/1 of every year).
    • The cut off for the Flu Declination form to be used for each flu season is August 28.
      • If form is signed prior to 8/28, this form expires 10/1 of the current year until a vaccine is received.
      • If the form is signed after 8/28, the form will then expire on 10/1 the following year.
      • Should the candidate provide flu vaccine documentation, the Flu Declination form would be moved to expired upon one year from the date of their signature on the Flu Declination form.
  • Completion of this form does not mean you are saying NO to receiving the flu shot….it can also be used as a placeholder (as described above) for Atlas/Facilities to cover if you are choosing to delay obtaining the vaccine. Facilities may reject the usage of a declination; your compliance team will inform you of this if this is the case. Please communicate with your recruiter and/or compliance team ASAP if you are choosing to decline the vaccine for any reason.
  • If you intend to get the shot, you can simply check the OTHER reason for declination and put a comment that you will get it later in the season OR use the General Declination reasoning if you prefer.
    • Again, this doesn’t mean you can’t or won’t be getting the vaccine it is a general form to cover any gaps.

 

Uploading Instructions

  • To upload all credentialing and health documentation access your My Atlas Access
    • If on your task list, simply upload to that line item.
    • If you do not see it on your task list, you can go to my profile > credentials > update if previously uploaded.
      • If neither of the above applies, go to my profile > credentials > add credential and choose flu vaccine
    • You are only able to upload one document. Therefore, if you have a 2-3 page document, it must be compiled into one saved item.
  • Username = email address linked with Atlas MedStaff profile

 

Insurance for Flu Vaccination

  • If you have Atlas MedStaff insurance, BCBS will cover the cost of your flu shot
  • You can obtain your flu shot at Walgreens, Costco, Walmart, etc. To find a location near you, login to your MyNebraskaBlue Account and scroll to Pharmacy Benefits
  • Atlas MedStaff insurance does not apply at CVS pharmacies

Frequently Asked Questions

Q. Why do I need to send flu documentation if I completed the attestation form?

A.

  1. Are you truly needing to decline or just buy time? Buying time – there’s a high chance your current facility doesn’t accept declinations (ask your compliance specialist for further information).
  2. Was the Flu Attestation (aka Flu Decline form) signed prior to 8/28 without providing a new flu vaccine for the coming Season?
    • If Yes, that means that declination form expires as of 10/1 current year.
    • With a vaccine on file, flu declinations expire 1 year post signature.
      • EXAMPLE: Annual Flu Decline signed 7/15 with no coming year Vaccine on file
        • Flu Decline expires 10/1 of current year and an updated one will be asked for signature.
        • Once the coming year vaccine is provided, the flu decline will be adjusted to reflect expiration from 1 year of date signed.

Q. I need to medically decline. Do I need a doctor’s note?

A. For Atlas MedStaff, a physician note is not required. However, many of our clients will ask for a physician’s letter as supporting documentation.

Q. I need to decline for religious reasons. Do I need a clergy note?

A. For Atlas MedStaff, this is not required. However, many of our clients will ask for a clergy letter as supporting documentation.

Q. Can a facility reject my request for exemption?

A. Yes. While Atlas will accept your declination, any of our clients have the ability to reject your exemption and Atlas will not be able to provide an exemption.

Q. I only got the receipt, and it says administered. Why can’t you take this?

A. Many pharmacies will not provide additional documentation unless prompted to print. We cannot use Rx filled for proof of administration to the body. There are very specific Joint Commission and Client requirements we are expected to adhere to.

Q. I emailed you the document, why am I being asked to upload it too?

A. At Atlas MedStaff, we want to ensure that your experience with your AMS portal (ABLE) is always up to date. It can only track documents if they are uploaded to your portal. If you upload the items to your portal, then you can also retrieve them and get a copy for your records at any time.

Q. If I got the vaccination on-site/at facility, why do you not have record of it?

A. Due to HIPAA conflicts, we are not able to obtain your records from an on-site employee health department.  You will need to reach out to them to obtain the vaccine administration form to upload to your AMS portal.
