Job Description:

Join us as a Case Manager Registered Nurse in Oakland, California! This is a unique opportunity for skilled professionals who enjoy a fast-paced environment focused on inpatient discharge planning. As a backfill for the holidays, you’ll be integral in coordinating care transitions as patients move from the hospital to home or other settings. Your days will be filled with collaborative efforts across various inpatient units, ensuring that every patient receives the resources they need for a successful discharge. With your expertise of at least three years in acute care settings and experience using EPIC systems, you'll work closely with multidisciplinary teams to facilitate smooth transitions.

This position offers a structured schedule of 5 days a week, allowing you to enjoy your evenings and weekends while making a significant impact in patients' lives. Float assignments may occur every other week, adding a touch of variety to your work experience.
