Job Description:

Join our team as a Registered Nurse in Case Management, where you will take on a crucial role navigating the complexities of patient care. This exciting position allows you to work with a diverse range of inpatient adults across various units, including ICU, PCU, MedSurg, and ER. With a 4/10 shift schedule every other weekend, you’ll have the opportunity to balance work and life while contributing your expertise in acute care environments.

In this role, you'll leverage your strong background in case management to conduct clinical assessments, develop care plans, and ensure seamless post-discharge transitions for patients. Collaboration is key, and you’ll work closely with leadership, peers, and case management assistants to provide top-notch support.
