Job Description:

The RN Case Manager will focus on Care Coordination, Care Transitions, and Discharge Planning within the acute care setting. This role involves performing timely Initial Assessments, updating notes, and preparing final notes. The nurse will facilitate referrals to post-acute care services and collaborate with a multidisciplinary team to ensure smooth discharge processes. This assignment offers a chance to work in a vital role that directly impacts patient flow and recovery, with a balanced schedule that avoids overnight shifts. Job Specifics:

Location: San Francisco, CA
Shift: 8-hour days, Friday through Monday, 08:00 - 16:30
