Job Description:

As an Adult Nurse Case Manager, your workdays will be filled with proactive communication, where you’ll liaise with payors for treatment plan approvals and resource allocation. Use your clinical knowledge to collaborate with a multi-disciplinary team to enhance transition planning for patients, aiming for successful moves to post-acute settings. You’ll be navigating nursing facilities, rehabilitation centers, and local shelters—all in a day's work!
