Job Description:

Step into a critical role as a Registered Nurse Case Manager in a busy hospital setting, where you'll coordinate discharge planning and facilitate smooth patient transitions. This assignment is set on a dedicated unit with a mix of ICU, Observation, Telemetry, Progressive Care, Neuro, and Orthopedic patients, offering a well-rounded experience in patient case management. You'll work with a team of 15 RNs and 15 social workers, managing a patient load of approximately 30 per RN. With 8-hour days from 8:30 AM to 5:00 PM, Monday through Friday, this 13-week contract provides a straightforward schedule that allows you to focus on what truly matters — supporting patients and their families through often difficult conversations. Expect to employ Meditech or Midas EMRs and utilize modern office equipment as you ensure efficient care coordination. This role suits proactive, adaptable professionals who excel in communication and thrive in dynamic environments.
