Job Description:

Join a dedicated team of registered nurses in Juneau, Alaska, for an exciting travel contract as a Case Manager. In this role, you’ll be the linchpin in coordinating patient care within the Southeast Alaska Regional Health Consortium (SEARHC). Your days will involve collaborating closely with medical providers and interdisciplinary teams to develop personalized care plans, manage complex cases, and ensure seamless referrals and follow-ups. You'll also step into various nursing duties, including patient assessments, education, and medication administration, all while supporting specialty clinics, OB care, and video telehealth visits.
