Job Description:

This contract position involves working as a case manager in a hospital setting, primarily focusing on coordinating patient care, conducting face-to-face interviews with patients and families, and reviewing patient charts daily. You will utilize MediTech and Midas EMR systems to perform discharge planning, InterQual reviews, and daily multidisciplinary rounds. The role offers the opportunity to engage directly with a diverse patient population on the first floor of a busy hospital across from the cafeteria, requiring a proactive and personable approach. Expect to handle a caseload with a typical patient ratio of 1:40 during standard daytime hours, with an emphasis on efficient documentation and resource coordination. The assignment is ideally suited for a personable, adaptable professional comfortable navigating a fast-paced hospital environment.

Shift: 8-hour days, rotating weekends (Saturday and Sunday, 8am-4:30pm)
