Job Description:

Join our team as a hospital Case Management RN in Vicksburg, MS, and take on a challenging and rewarding contract role. This position focuses heavily on discharge planning, utilization review, and patient advocacy within an acute hospital setting. You'll need at least three years of hospital case management experience, with expertise in managing patient transitions to home health, skilled nursing facilities, LTAC, and IRF. Your day will involve coordinating with multidisciplinary teams, preparing patient choice letters, and ensuring compliance with regulatory requirements—all while working with Cerner, MorCare, and CarePort systems. This assignment requires a hands-on professional who can hit the ground running from day two and bring a wealth of hospital case management knowledge to the table. The role is vital for ensuring seamless patient care transitions, quality resource utilization, and adherence to hospital policies.
