Job Description:

This inpatient case management RN role offers a rewarding opportunity to coordinate medical home care and facilitate safe discharges within a hospital setting. Working primarily during daytime hours, you'll handle approximately 15 inpatient cases daily, navigating social issues, patient placement, and wrap-around services. The position emphasizes inpatient hospital or acute care experience, with EPIC charting preferred—so if you've ever navigated the Epic system or managed complex cases, this job is right up your alley. You'll enjoy a steady 40-hour week with Monday through Friday shifts, and occasional floating between facilities located in Bel Air and Aberdeen, Maryland, within a 10-mile radius.
