Job Description:

Join our team as a Manager of Case Management in Falls Church, Virginia, and take on a leadership role in overseeing patient care coordination within a hospital setting. This contract position offers an opportunity to lead interdisciplinary teams in discharge planning, transitions of care, and clinical performance improvement initiatives. You'll supervise staff, ensure timely referrals and discharge processes, and collaborate closely with physicians, patients, and families to facilitate smooth care transitions. This role provides a balanced mix of managerial oversight and hands-on process evaluation, making each day both meaningful and varied.
