Job Description:

Join a busy case management team at a reputable hospital in Lone Tree, Colorado, where you'll focus on coordinating discharge planning, patient assessments, and working closely with physicians, families, and community services. This role offers a chance to be involved in high-impact activities such as arranging medication follow-ups, transportation, and ensuring smooth patient transitions. The work takes place on the hospital’s 5th floor in a 45-bed unit, primarily during the day shift from 8:00 AM to 4:30 PM, with some weekend coverage. Expect to manage a caseload of 18-25 patients per case manager while utilizing Meditech EMR and CarePort systems.
