Job Description:

Join the team at Johns Hopkins Hospital as a Transplant Nurse Coordinator in Baltimore, MD. This role offers a unique opportunity to be at the forefront of heart transplant care, coordinating complex procedures and supporting patients through their transplant journey. You'll be responsible for managing on-call activities such as organizing transplants and triaging patient concerns via telephone. The position involves preparing patients for transplantation, arranging and monitoring testing, providing education, and collaborating closely with multidisciplinary teams like pharmacy, social work, and nutrition. Expect to review charts, manage medication refills, and ensure seamless communication across care providers. The schedule is flexible, with three days per week on-site, working in person during clinic hours, and attending to critical responsibilities in a stimulating and impactful environment.
