Job Description:

This 13-week travel assignment involves working as a Care Coordinator in a facility that emphasizes seamless patient transitions and efficient resource utilization. You will coordinate and facilitate care plans, collaborate closely with the Interdisciplinary Care Transitions (ICT) team, and support patients throughout their continuum of care. Your role requires assessing patient needs, developing comprehensive care strategies, and ensuring compliance with external review agencies. Expect to play a vital part in improving patient management and satisfaction, all while navigating discharge planning and utilization review processes in a fast-paced environment that appreciates your expertise.
