Job Description:

This contract position calls for a skilled case management professional to join a large, busy hospital in Boston. You'll be working 10-hour day shifts from Monday to Friday, focusing on discharge planning and case coordination in an acute care setting. The ideal candidate brings at least 2 years of case management experience within a hospital environment, preferably in a teaching hospital with over 500 beds.
