Job Description:

This RN position in case management takes you to a busy hospital setting where you'll coordinate patient care plans, facilitate discharge processes, and collaborate with a dedicated team of nurses and social workers. You'll work a standard 8-hour day shift for a 13-week contract, engaging in complex case reviews, resource coordination, and documentation using Meditech, Midas, and Wellsky systems. The role offers a chance to navigate a vibrant hospital floor with a 289-bed capacity, handling a patient ratio of 1:25 for RNs and 1:40 for social workers.
