Job Description:

This travel contract is for a Case Manager Registered Nurse who will assess patients, evaluate their progress, and guide discharge planning. Responsibilities include coordinating services at the appropriate level of care, ensuring treatment plans are initiated and reviewed daily, monitoring length of stay, and identifying barriers that may delay discharge. The role also includes following Medicare requirements related to patient choice, IMM notices, and MOON notices. This Monday-Friday assignment offers consistent daytime hours with no weekend or call requirements. Nurses should have at least three years of experience, as first-time travelers are not accepted for this assignment.
