Job Description:

We are seeking a skilled Case Manager with at least two years of hospital experience and proficiency in Midas, Careport, and Meditech. In this role, you will play a crucial part in the inpatient hospital setting, focusing on developing discharge plans, performing Interqual reviews, and managing patient admissions and transitions to post-acute care.

As part of a dedicated team, you'll work 8-hour days, where your organizational skills and clinical knowledge will shine.
