Job Description:

Join our team as a travel healthcare professional in Baltimore, Maryland! We are seeking an experienced RN Case Manager to coordinate care for a select caseload of patients in an adult medicine/surgery setting. This unique role offers you the opportunity to work in a dynamic hospital environment with a focus on effective care transitions. You’ll engage with multidisciplinary teams and play a crucial role in ensuring patients receive the necessary resources for their treatment plans.

In this role, you will work five 8-hour shifts from 8:30 am to 5 pm, Monday through Friday, allowing you to enjoy your weekends and holidays free! You’ll navigate the complexities of case management, from triage to safe transitions to home or post-acute settings. Your expertise in EPIC and familiarity with post-acute facility referral platforms will be invaluable as you actively coordinate care with payors and maintain meticulous patient records in compliance with operational guidelines.
