Job Description:

As an RN with case management expertise, you'll oversee patient discharge planning, resource utilization, and holistic needs assessment in a setting that emphasizes efficiency and quality. The work involves clinical assessment, collaborative planning, and fostering communication across multidisciplinary teams, all within a Monday through Friday schedule from 8 a.m. to 4:30 p.m.

Location: Dover, New Jersey
Shift: 8-hour days, Monday - Friday, 8:00 a.m. – 4:30 p.m.
