Job Description:

Join us in a vital role supporting patients through the care coordination process in an outpatient setting. You'll work closely with healthcare teams to ensure a smooth transition for patients moving between inpatient and outpatient services. Your focus will be on maximizing health outcomes, providing clear patient and family education, and coordinating care plans that promote trust and advocacy. This role involves managing care transitions, assisting with pre-visit planning, reconciling medications, and ensuring all documentation and follow-up are completed efficiently. The ideal candidate will bring a strategic mindset to patient advocacy, with an eye for detail and a passion for fostering positive patient experiences.
