Job Description:

This case management contract offers an opportunity for a dedicated healthcare professional to step into an acute care hospital setting where your expertise will truly make a difference. You will be floating between two nearby campuses, Walnut Creek and Concord, coordinating patient transitions and ensuring seamless care coordination during your shifts. The role involves working 8-hour days, primarily from 0800 to 1630, on a set schedule that may include every Tuesday through Saturday or a rotating pattern. This assignment provides a dynamic environment where your acute care experience will be essential as you navigate complex cases and collaborate with hospital teams.
