Job Description:

Join our team as a Case Manager Float, providing essential support across multiple acute care hospital settings. This role involves navigating the fast-paced environment of both Walnut Creek and Concord campuses—just eight miles apart—ensuring each patient receives the appropriate case management. You will work on a set rotation, with shifts from Tuesday through Saturday, focusing on coordinating care and facilitating smooth patient transitions. Your expertise in acute care case management will be put to good use as you handle complex cases, all while experiencing the unique rhythm of each facility.
