Job Description:

Join us as a dedicated Care Manager II in a contract role that puts you at the heart of patient transitions and discharge planning within an acute hospital setting. Your expertise in case management, utilization review, and familiarity with MCG, InterQual, and EPIC will be essential as you coordinate care from admission to discharge. This position offers an engaging opportunity to support patients and their families through comprehensive assessments, effective resource utilization, and collaboration with multidisciplinary teams. You'll play a key role in orchestrating smooth patient flow, addressing barriers to discharge, and advocating for optimal health outcomes—all while working a predictable 8-hour shift in beautiful Longview, Texas.

Job Specifics:

Location: Longview, Texas
Shift: 8 hours per day, generally 5 days a week
Duration: 13 weeks total, with a weekly commitment of 40 hours
Hourly Rate: $70 per hour
Weekly Gross Pay: approximately $1,716.41
