Job Description:

This RN case management assignment takes place in an acute hospital setting in Clinton, Maryland. The role involves discharge planning and coordinating care for up to 20 patients daily, requiring a proactive approach to ensure smooth transitions from hospital to other care settings. You'll be floating across Medical units, ICU/CCU, and Maternal Child departments, providing support where needed. This is a weekday schedule with 8-hour days, offering consistent hours and a chance to make a tangible impact on patient outcomes. The ideal candidate is comfortable working independently, managing a diverse caseload, and possessing a Maryland RN license with at least two years of recent hospital experience.
