Job Description:

Join a vibrant acute care setting as a registered nurse specializing in case management and discharge planning. In this role, you'll coordinate patient transitions, ensuring timely and efficient discharges while managing a caseload of approximately 20 patients during daytime shifts. This assignment offers a balanced work schedule with five 8-hour days, including every other weekend and holiday coverage, providing ample opportunity for work-life harmony. Expect to bring at least two years of acute care experience, and you'll be expected to work in business attire or scrubs while embracing an environment that values professionalism and compassionate, patient-centered care.
