Job Description:

Join a dedicated team of RN Case Managers at St. Davids Round Rock Medical Center for a 13-week contract in case management. This role involves coordinating care plans, discharge planning, and facilitating smooth patient transitions within a busy hospital setting. You'll work daytime hours, Monday through Friday, 8:00 AM to 5:00 PM, with no call requirements. The unit supports a 209-bed facility with a team of 27 staff members, including RN Case Managers and Social Workers, all committed to delivering high-quality patient care through effective care coordination. The environment offers the opportunity to utilize your clinical knowledge and case management expertise while working with advanced EMR systems like Midas, Meditech, Interqual, and Care Port. This assignment provides a chance to make a tangible difference in patients' lives while experiencing the vibrant community around Round Rock, Texas.
