Job Description:

Join our team as a Clinical Liaison at the El Camino Acute Rehab Unit, located on the campus of Los Gatos Hospital. In this role, you'll be the connective tissue between inpatient rehabilitation services and the community, helping coordinate care for adult and geriatric patients with conditions like stroke, brain injury, and spinal cord injury. You'll leverage your knowledge of post-acute care, care coordination, and business development to build relationships and ensure smooth patient transitions. With a schedule of weekday 8-hour shifts, this position offers a chance to make a meaningful impact in a setting equipped with an inpatient rehab gym and EPIC electronic health records. Flexibility is key, as some local travel to neighboring facilities may be required to ensure all patients receive the attention they need.
