Job Description:

Join a dedicated team as a registered nurse specializing in care coordination and patient transitions within an acute care setting. In this role, you'll collaborate closely with physicians, social workers, and the interdisciplinary team to ensure patients move through their care journey smoothly, preventing unnecessary readmissions. This position offers the opportunity to focus on optimizing patient health outcomes, managing care resources effectively, and supporting patients and their families during critical moments—all while working four 10-hour days, including weekends, in a fast-paced hospital environment that values teamwork and efficiency.
