Job Description:

Join a case management role focused on the complex health needs of patients with diabetes or kidney disease. This assignment involves coordinating care for individuals with chronic conditions, conducting transition of care calls, providing pre-dialysis education, and monitoring treatment adherence. You'll work closely with interdisciplinary teams—including RNs, LPNs, ICMs, and nephrologists—to ensure seamless patient support. This role offers hands-on experience with CKD progression, dialysis modalities, vascular access, and fluid management, making it a unique opportunity to deepen expertise in population health within a collaborative setting.
