Job Description:

Join us as a Care Management RN at Lutheran Hospital, where you'll be the captain of discharge planning and patient transition coordination within the hospital setting. This 13-week contract offers an engaging opportunity to utilize your four-plus years of experience in care management, working with a patient ratio of 1:20. Your expertise in discharge processes and hospital procedures will help ensure smooth patient journeys. The shift runs from 8:00 am to 4:30 pm, with holidays required during the contract period. Dress code is business casual or black scrubs, and Cerner EMR is used for documentation. This role involves working primarily in the care management department, with some flexibility to float to other units if needed. You'll be based at our facility in Fort Wayne, Indiana, and must be more than 50 miles from the hospital to qualify.
