Job Description:

This registered nurse position as a Case Manager places you at the forefront of patient discharge planning and coordination within an acute care setting. You'll be managing a caseload in a facility that values precise and compassionate clinical oversight, working 8-hour daytime shifts from 08:00 to 16:30. The role involves evaluating patient progress, facilitating secure transitions from hospital to community, and collaborating closely with healthcare teams to ensure quality care.
