Job Description:

Join our team as a Case Manager Registered Nurse in Leominster, MA, where you'll play a vital role in discharge planning within a community hospital setting. This position involves conducting initial assessments and coordinating patient discharges, primarily focusing on cases with minimal utilization review involvement thanks to a dedicated team. You'll enjoy working with a manageable patient load of approximately 15-25 patients per day, allowing for meaningful interactions and effective discharge planning. The role offers flexible daytime hours—either 7 AM to 3 PM or 9 AM to 5 PM—across a 13-week assignment, providing a balanced work schedule.
