Job Description:

Join our team as a Registered Nurse in Case Management at a prominent hospital in Oakland, California. This 13-week contract offers the opportunity to lead patient care coordination and transition planning in an inpatient acute care setting. You'll be working in collaboration with physicians, social workers, and the interdisciplinary team, ensuring patients move through their care journeys smoothly and efficiently. Your expertise in inpatient case management will be pivotal in preventing unnecessary readmissions and optimizing resource utilization. The shift runs five days a week, 8 hours per day, with a schedule from 08:00 to 16:30. This role combines clinical assessment, care coordination, and strategic communication, making each day both challenging and rewarding for a dedicated RN with inpatient acute care experience.
