Job Description:

Join us as a Case Manager for Population Health on a night shift in Detroit, MI. In this role, you'll be a key player in a patient-centered care team, focusing on assessment, planning, and care coordination for individuals navigating complex health challenges. Your days will involve conducting comprehensive evaluations of patients’ biomedical, psychological, and social needs, developing tailored care plans, and engaging families as active partners.
