Job Description:

Step into a pivotal role supporting patient transitions and discharge planning at a prominent healthcare facility. As a registered nurse in case management, you'll coordinate care for patients across various units, including ICU, MS, PICU, SDU, and more. Your days will involve assessing patient needs, managing discharge processes, ordering DME, and ensuring compliance with regulatory standards—all while working a consistent schedule of weekday dayshifts. Location: Greensboro, North Carolina
