Job Description:

Join a dedicated team as an acute care RN Case Manager in Salem, MA, where you'll be vital in coordinating inpatient medical and surgical patient discharges. This role involves managing a patient load of approximately 15 adults on EPIC charting systems, working standard daytime hours, Monday through Friday. Your experience in utilization management, discharge planning, and acute care hospital settings will help ensure efficient patient transitions.
