Job Description:

Join our team as a Case Manager in the Emergency Department at a busy Chicago hospital. In this role, you will support adult and geriatric patients, many facing complex social and health challenges, including unhoused and low-income populations. The position involves managing a caseload of approximately 20 patients, assisting with insurance navigation, connecting patients to community resources, and collaborating with a multidisciplinary team. While previous ED case management experience is a plus, training will be provided upon arrival. The ideal traveler is an attentive assessor, a resourceful problem-solver, and a team player eager to make a real impact in an underserved community.
