Job Description:

Join us as an RN Care Manager in Lebanon, NH, where you'll oversee inpatient caseloads with a focus on post-discharge planning. This role involves conducting initial assessments, developing coordinated care plans with patients and their families, and collaborating closely with medical teams during interdisciplinary rounds.
