Job Description:

Join us as a Case Manager Registered Nurse in a short-term acute care setting where you'll support patients by coordinating care plans and ensuring smooth transitions between departments. You'll work in a facility with 108 beds, floating between units as needed, and be part of a dedicated team utilizing Epic charting software. This role involves 8-hour day shifts, and you will experience a variety of clinical environments within the hospital.
