Job Description:

Join us as a Case Manager RN in Bel Air, Maryland, where you'll be overseeing complex patient cases that involve guardianships, medical placements, and social situations. This role offers the chance to coordinate inpatient hospital and acute care discharges, manage social wrap-around services, and ensure patients transition safely back to their communities. You'll spend your days working in a collaborative, business casual environment, charting in EPIC, and engaging with approximately 15 cases daily. The position provides an engaging mix of administrative oversight and patient coordination, with opportunities to float between facilities within a 10-mile radius.
