Job Description:

This 13-week contract at HCA Florida Oak Hill Hospital offers a hands-on experience for Medicaid case managers working in a busy hospital setting. You'll coordinate and support patient transitions, navigate electronic systems like Meditech and Midas, and collaborate with a dedicated team of social workers and case management staff. The role involves managing acute cases within the hospital's 350-bed facility, primarily during eight-hour day shifts, with a focus on efficient patient throughput and discharge planning.
