Job Description:

Join a rewarding contract as a Case Manager Registered Nurse in Santa Rosa, California, where you'll coordinate utilization management, discharge planning, and post-acute care referrals to ensure smooth patient transitions. You'll oversee complex acute patient populations, focusing on providing coordinated and integrated care that helps prevent unnecessary readmissions. Expect to work 8-hour days, with shifts typically from 8:00 AM to 4:30 PM, including a minimum of two weekend shifts per month.
