Job Description:

This contract role focuses on the coordination and management of patient care in a long-term acute care (LTAC) setting. As a case management professional, you will oversee resource utilization, develop comprehensive care plans, and support smooth discharge planning to ensure patients receive personalized, efficient care. You'll act as a patient advocate, ensuring that treatment aligns with patient needs and preferences while collaborating closely with interdisciplinary teams. Job Specifics:

Location: Indianapolis, Indiana
Shift: 8-hour days, Monday - Friday, 8:00 am - 5:00 pm
