Job Description:

Join a dedicated team as a Manager overseeing the RN/CM unit in a facility that values leadership and expert case management. This role offers the chance to supervise a group of experienced professionals, including RN Case Managers and Social Workers, in a setting that combines clinical excellence with a collaborative spirit. You’ll be guiding the discharge planning process, managing a caseload that varies from 15 to 30 patients depending on staffing, and coordinating care across multiple departments. The unique aspect of this assignment is the small hospital environment, where your leadership will directly impact patient outcomes and team cohesion. Expect a schedule of four 10-hour days with every other weekend rotation, allowing for a balanced work week while engaging in meaningful case management that supports diverse patient populations in NICU, pediatric, ER, MedSurg, Tele, Oncology, and ICU units.
