Job Description:

Join a rewarding utilization review and swing bed case management role in Portales, NM, where you’ll coordinate discharge planning, evaluate patient admissions, and work closely with physicians and healthcare teams. This position offers the chance to oversee swing bed placements, manage documentation including MDS, and facilitate seamless transitions for patients requiring various levels of care.
