Job Description:

This RN Case Manager role at Winchester Medical Center offers a rewarding opportunity to coordinate patient care within a midsize facility known for its safety and trauma designation. You'll manage a caseload of Medicare, Medicaid, commercial, and unfunded patients, overseeing case planning, resource facilitation, and discharge coordination. Work predominantly during daytime hours, with potential weekend and on-call responsibilities. The position requires at least two years of RN Case Management experience, a VA State license or Compact, and BLS certification. EPIC EMR proficiency is a must.
